Year 3 Schedule Preference Questionnaire

Below are the schedule options for each rotation. If you have any preferences please rank each option. If you have no preference, that's fine; you can check that box or leave the question unanswered. Surgery rotation placement is determined by the clerkship director and will be sent to you separately (please note that you may be placed in JAX or GNV but that has no bearing on your other placements). Please be sure to provide your gender and preferred scrub size at the end of the survey!
  • Pediatrics Clerkship: Please consider the dates of your clerkship based on your rotation group assignment; and rank the following schedule choices:

  • Family Medicine and Neurology Clerkships: Despite being scheduled together, these are two distinct clerkships. This 12-­week period is divided into three 4-­week blocks (two sections of Family Medicine/Ambulatory Care and one section of Neurology). Please consider the dates of your clerkship based on your rotation group assignment, and answer sub-­questions a. and b.
